Job Description
In this role, you will provide protected, evidence-based nursing care and support optimal health outcomes for residents and their families across Dhelkaya Health services.

Key responsibilities

- Undertaking resident assessments and reporting clinical findings or changes to the Registered Nurse.

- Contributing to the development and implementation of person-centred care plans.

- Delivering direct care in line with individual resident needs, goals, and preferences.

- Monitoring and evaluating care outcomes in collaboration with the Registered Nurse and care team.
