Aveo Group is seeking a Care Partner (Clinical) to support residents in our Aveo communities. You will assess care needs, create personalised plans, and coordinate services to help residents live independently at home.

This role emphasizes compassionate communication, collaboration with GPs and allied health, and using the Alayacare system. Ideal candidates are RNs with home care experience, solid customer service, and a valid driver's license.
