In this Part time position, you will play a crucial role in providing personalised care and support to participants located in their home and in the community. As a key member of our compassionate team, you will have the opportunity to make a real difference in the lives of our clients while enjoying the stability of permanent part time or full time work.

What you'll be doing

n

n
- Assisting clients with daily living activities such as personal care, meal preparation, and household tasks
n
- Facilitating community access and engagement to support clients' independence and social inclusion
n
- Implementing individualised support plans to address clients' unique needs and goals
n
- Collaborating with allied health professionals and family members to ensure holistic, person-centred care
n
- Maintaining detailed client records and documentation in line with NDIS requirements
n
- Adhering to all relevant policies, procedures, and safety standards to ensure the wellbeing of clients
n

What we're looking for

n

n
- Applicants MUST have their own vehicle available for all shifts and fully insured
n
- Applicants MUST be available for shifts outlined in the screening questionaire
n
- Relevant Certificate III or IV qualification in Disability, Aged Care, or Community Services preferred
n




- Previous experience providing support to individuals with disabilities or complex needs mental health experience is essential
n
- Minimum of 1 year experience providing support to individuals with disabilities or complex needs is required
n
- Ability to manage Medication
n
- Assistance with self care
n
- Strong communication and interpersonal skills, with the ability to build rapport and trust with clients
n
- Empathetic, patient, and adaptable approach to working with diverse client needs
n
- Ability to work independently as well as collaborate effectively within a team
n
- Proficient in documentation and report writing
n
- Current First Aid certification and a valid driver's licence
n
- Australian work rights are required for this position
n
- Both NDIS Worker Check and Working with Children Check are required
n
