Job Description

Western Health in Melbourne is seeking a Community Mental Health Case Management clinician to deliver recovery-focused, person-centred care to a caseload with complex needs. You'll assess, plan and coordinate services in a quick-paced community setting.

You will work with families, carers, and the multidisciplinary team, maintain documentation, monitor risk, and apply the Mental Health and Wellbeing Act 2022.
