Support at Home Clinical Care Partner (Registered Nurse)

Cobden Health is seeking a dedicated Clinical Care Partner (Registered Nurse) Part-time/Casual to join our growing Support at Home team. This role provides the chance to support older people to remain independent, safe and well in their own homes while delivering high quality, person-centred care. The position is responsible for clinical assessments, care planning, care coordination and ongoing case management for clients receiving Support at Home services.

About the role

You will:

- Conduct client assessments and reviews
- Develop and monitor individualised care plans
- Coordinate services and support providers
- Work closely with clients, families, GPs and allied health professionals
- Monitor clinical risks and support positive health outcomes
- Ensure funding, documentation and compliance requirements are met.

About you

You will be:

- A Registered Nurse with current AHPRA registration
- Passionate about supporting older people to live independently
- Highly organised with excellent communication skills
- Able to work autonomously and as part of a team




- Committed to providing exceptional customer service and quality care.
- Able to Work in Australia without any working right restrictions
