About us:

Cairns and Hinterland Hospital and Health Service provide an extensive range of health services at more than 30 regional, rural and remote facilities across a geographical area of 142,900 square kilometres. The Health Service is 95 percent self-sufficient with only a small number of high-level acute services being provided in Townsville and Brisbane.

About your opportunity:

As the Diabetes Nurse Navigator, you will play a pivotal role in enhancing patient care within the diabetes specialty. This position is designed to complement existing healthcare models while embodying the principles of Nurse Navigation. You will be accountable for delivering advanced, patient-centred care through effective facilitation and coordination of complex diabetes cases.



The Nurse Navigation model fosters a team-oriented, cross-sectoral approach that prioritizes the individual needs of patients navigating their recovery journey.

About your role:

- Enhance patient quality of life through the application of evidence-based patient centred care
- Provide strong leadership to motivate the patient and multidisciplinary team to achieve agreed care pathway planned goals within designated timeframes.
- Facilitate effective patient centred care by enhancing systems integration, reducing fragmentation and barriers, and establishing a personalised care pathway – for example, using Telehealth or other technology to facilitate care coordination.
- Lead quality improvement activities and participate in research activities relevant to the assessment and development of the role on an ongoing basis. This may take the form of collating outcome measures, key performance indicators, staff and patient satisfaction surveys, or any health service improvement activity.
- Facilitate learning opportunities to support the development of contemporary nursing practice skills, knowledge and professional capabilities of self and others.
