Ambition Health Group is seeking an experienced SIL Team Leader to join our growing Accommodation & Housing team. Reporting directly to the Accommodation Coordinator, you will manage an allocated portfolio of existing SIL participants and homes, providing hands‑on leadership to support workers while ensuring participant services, documentation, stakeholder communication and compliance remain at a high standard. This role is ideal for an experienced SIL Team Leader, House Manager, SIL Coordinator or senior disability skilled ready to take ownership of a participant caseload.

Key responsibilities

- Manage an allocated caseload of existing SIL participants and homes.
- Maintain regular contact with participants, families, Support Coordinators, Behaviour Support Practitioners, allied health professionals and other care‑team members.
- Coordinate and participate in care‑team meetings, including agendas, minutes, action plans and follow‑up.
- Conduct regular 1:1 supervision meetings with Disability Support Workers, providing coaching, feedback and practice guidance.
- Ensure participant Service Agreements, support plans, risk assessments, Behaviour Support Plans and other documentation remain current and complete.
- Monitor the quality of progress notes, incident documentation and other SIL records.
- Conduct regular SIL house visits and identify participant, staffing,



documentation or operational concerns.
- Identify participant‑specific training and competency requirements and coordinate training with clinical and external professionals.
- Monitor incidents, risks, safeguarding concerns and changes in participant needs, escalating significant matters appropriately.
- Support new participant transitions, including house readiness, discharge planning, staff training and smooth handover into ongoing SIL support.

About you

- Minimum 2 years' experience within NDIS/disability services.
- Strong hands‑on experience within Supported Independent Living (SIL).
- Previous experience leading or supervising Disability Support Workers.
- Experience managing participant documentation and service requirements.
- Confidence working with families, Support Coordinators and multidisciplinary care teams.
- Experience conducting staff supervision, coaching or 1:1 meetings.
- Good understanding of the NDIS Practice Standards and Code of Conduct.
- Strong written communication, organisation and follow‑up skills.
- Ability to confidently manage multiple participants, staff and priorities.
