By bringing together clinical and administrative teams, the CCSC delivers culturally responsive care coordination that improves access to care, enhances patient experiences and supports better health outcomes across the Torres and Cape and Cairns and Hinterland regions.**Identified Role:** Under Section 25 of the Anti-Discrimination Act 1991 (Qld), it is a genuine occupational requirement for the incumbent to be Indigenous to the Aboriginal or Torres Strait Islander Community.**Skilled and personal motivators!**As a professional working within Torres and Cape HHS,
