About the Role

Monash Health Aged Care Services is seeking a passionate and experiencedCase Managerto join ourCommunity Support Options team. Based within ourHome Care Package (HCP) program, you will play a key role in supporting older people and their carers across theCasey, Cardinia, Greater Dandenong, Kingston, and Bayside municipalities. This is an exciting opportunity to contribute to high-quality, person-centred care in line with the latest aged care reforms.

Key Responsibilities

- Deliver awellness and reablement-focused model of care, managing a diverse caseload of HCP clients.




- Conductcomprehensive client assessmentsand develop tailored care plans to enable independence at home.
- Provide holistic support within a structuredcase management framework.
- Work collaboratively with rostering teams, agencies, contractors, and other stakeholders to ensure seamless service delivery.
- Actively contribute to theongoing development and improvementof the Home Care Program, including service planning and review.
- Monitor program outcomes by reporting onKPIs and budget performanceto Care Managers and Team Leaders.
- Coordinate with multiple service providers to ensure services remain aligned with individual care plans.
- Regularly review client needs, well-being, and progress, recommending adjustments to services as required.
- Manage new inquiries, referrals, and client information via theMy Aged Care Services Portal.
- Proactively resolve client-related challenges with practical, client-centred solutions.
- Stay informed onaged care reforms, legislation, and quality standards, applying this knowledge in practice.
- Prepare, manage, and monitorindividual client budgets.
- Support clients through the current transition to theSupport at Home Program, ensuring continuity of care.

About You

- Relevant tertiary qualifications inhealth,



social sciences, or a related discipline.
- Demonstrated experience incase management, ideally within aged care.
- Strong knowledge ofaged care reformsandconsumer-directed care models.
- Highly developedcomputer literacy, communication, and organisational skills.
- Ability to work bothindependently and collaborativelywithin a team.
- A commitment to deliveringhigh-quality, client-centred carewith empathy and professionalism.
