Phoenix Specialised Youth and Disability Services Pty Ltd is a leading provider of skilled Youth and Disability support staff and NDIS services across Victoria. Due to continued growth, we are seeking an experienced and motivated Level 2 Support Coordinator to join our team, supporting participants across the NDIS, Child Protection, and Forensic Disability sectors.
This role requires a proactive qualified who can work both collaboratively within a team and independently to deliver high-quality outcomes.

Key Responsibilities
- Manage and support a caseload of approximately 20 participants
- Develop and implement effective case plans
- Assist participants and their families to understand and utilise their NDIS plans
- Facilitate care team meetings with participants and key stakeholders
- Build participant capacity and promote independence
- Monitor plan budgets and outcomes to ensure effective utilisation of funding
- Develop and maintain professional relationships while upholding appropriate boundaries
- Liaise with mainstream and community services (e.g. Housing, Education, Justice, Health) to ensure obligations are met
- Maintain accurate and compliant documentation, including case notes, reports, and billing records
- Ensure compliance with relevant legislation, standards, and internal frameworks
Skills and Experience
- Demonstrated experience working with people with disability
- Background in community services, disability, mental health, or related sectors
- Strong understanding of NDIS Practice Standards, Work Health and Safety, and Equal Opportunity principles
- Ability to problem-solve, show initiative, and achieve positive participant outcomes
- Knowledge and understanding of Aboriginal and Torres Strait Islander cultures (highly regarded)
- Strong knowledge of local disability, mainstream, and community services, with proven networking ability
- Experience within the Justice system (desirable but not essential)
- Excellent organisational and time management skills
- High-level verbal and written communication skills
- Experience in person-centred planning and service delivery
- Sound computer literacy, including report writing and documentation
- Knowledge of the NDIS and relevant legislation, including the NDIS Act 2013 and Disability Act 2006
Qualifications
- Certificate IV in Disability, Community Services, or a related field (highly regarded but not essential)
- Experience within the NDIS and disability sector (preferred)
Essential Requirements
- Current Driver’s Licence
- Valid Working with Children Check (Employee)
- Current National Criminal History Check (within the last 6 months)
- NDIS Worker Screening Check
- Evidence of Australian working rights
- Current First Aid and CPR certification
- NDIS Orientation Certificate
- Evidence of COVID-19 vaccination (including booster) or valid medical exemption
