Role Description This is a full time, on-site Community Support Worker role based in Perth, WA.

The Community Support

Worker will support clients with daily living activities, personal care, and household tasks in home and community settings.

Responsibilities include assisting clients to access community resources, participating in social and recreational activities, and promoting independence and inclusion. The role involves monitoring client wellbeing, documenting observations, and collaborating with families, care teams, and other service providers to follow individual support plans.

Worker will also help coordinate appointments, provide transport where required, and uphold safety, confidentiality, and compliance with organizational policies.

Qualifications

- Strong Interpersonal Skills and Community Support experience to build trust and provide person-centered assistance.
- Effective Communication skills (verbal and written) to engage with clients, families, and multidisciplinary teams.
- Knowledge of Case Management practices and Community Health principles to support and coordinate client care.
- Relevant qualification in community services, disability support, social care, or a related field (or progress toward such qualifications).
- Experience working with people with disability, older adults, or individuals with diverse support needs.
- Ability to follow individualized care plans, maintain accurate records, and work both independently and as part of a team.
- Current driver’s license and ability to travel locally as needed; appropriate clearances and checks as required by local regulations.
