The Opportunity

We are seeking a passionate and qualified Diabetes Educator to join our Chronic Care team. Reporting to the Chronic Care Manager, you will provide culturally safe, evidence-based diabetes education and care across Sunrise communities through regular outreach visits.

You’ll work closely with individuals, families and community groups to support diabetes self-management, early intervention and improved long-term health outcomes. Working as part of a multidisciplinary team, you’ll also contribute to coordinated care planning and build local capacity in diabetes management across remote communities.

Key Responsibilities

- Deliver high-quality, individualised diabetes care and education for patients and families at risk or living with diabetes and related conditions.
- Collaborate with multidisciplinary health teams to plan, coordinate, and implement effective chronic disease management.




- Provide clinical leadership and guidance to Remote Area Nurses, Aboriginal Health Practitioners, and allied health professionals.
- Support early intervention through patient referrals, evidence-based practice, and promotion of best-practice clinical standards.
- Travel regularly to remote SHSAC communities to deliver outreach services and strengthen local capacity in diabetes care.
- Maintain accurate clinical records, support quality improvement initiatives, and assist with QAAMS monitoring and training.

What You’ll Bring

- Current registration with AHPRA as a qualified health skilled
- Accredited Diabetes Educator (or eligibility to obtain credentialling within 12 months) with demonstrated experience in diabetes and chronic disease management
- Strong communication and interpersonal skills
- Capacity to work independently and manage a mobile workload
- Unrestricted driver’s licence
- National Police Clearance and OCHRE Card (or ability to obtain)
- Australian working rights
