NDIS Support Coordinator

Doncaster East, Victoria | Full-Time | 6-Month Fixed-Term Contract | $42–$45 per hour + superannuation

What Are We Looking For?

We are looking for an experienced and motivated NDIS Support Coordinator to join our team at our Doncaster East office on a 6-month full-time contract, with the possibility of extension. This role would suit an experienced Support Coordinator who is confident working independently, understands the NDIS landscape and has strong existing networks within the disability and community services sector.

We are particularly interested in someone who is well connected,



proactive and comfortable getting out into the community, building relationships and developing referral pathways that can connect participants with quality support coordination.

About the role

You will work closely with NDIS participants, their families and support networks to coordinate services, connect participants with appropriate providers and help them make effective use of their NDIS plans. Alongside managing your participant caseload, you will be encouraged to develop and maintain strong professional networks across the NDIS sector.

Your responsibilities will include

- Managing an allocated caseload of NDIS participants.
- Coordinating supports and services in accordance with participants' goals and NDIS plans.
- Building solid relationships with participants, families, providers and other stakeholders.
- Developing and maintaining relationships with support providers, allied health professionals, community organisations and other relevant networks.
- Actively engaging with your existing professional networks and developing new connections within the NDIS community.
- Identifying opportunities to connect eligible participants with our Support Coordination services.
- Assisting participants to understand and navigate the NDIS.
- Monitoring service delivery, participant needs and plan utilisation.
- Identifying gaps in supports and helping participants access appropriate services.
- Advocating for participants and supporting informed decision-making.
- Maintaining accurate case notes, records and documentation.
- Working collaboratively with allied health professionals, support providers and other stakeholders.
- Managing complex situations and escalating risks or concerns appropriately.




- Supporting participants to build capacity, independence and community connections.

About you We are looking for someone who has:

- Previous experience minimum 3 years working as a NDIS Support Coordinator.
- Strong knowledge of the NDIS and the participant journey.
- Established networks within the NDIS, disability and community services sector.
- A proactive approach to building professional relationships and developing referral pathways.
- Confidence getting out into the community and meeting providers, allied health professionals and other stakeholders.
- Robust communication, organisation and time-management skills.
- The ability to independently manage a caseload.
- Strong relationship-building and stakeholder-management skills.
- Excellent written documentation and case-note skills.
- A participant-focused and solutions-oriented approach.
- The ability to work autonomously while also contributing positively to a team.

Experience in Specialist Support Coordination and working with complex participants will be highly regarded.

What we offer

- $42–$45 per hour + superannuation
- Full-time, Monday to Friday.
- 9:00am–5:00pm.
- Based at our Doncaster East, Victoria office, with community and stakeholder engagement as required.
- 6-month fixed-term contract with the possibility of extension.
