We are currently seeking a full-time Care Partner to provide compassionate, person-centred care and support to our clients in their homes and communities. You will play a key role in helping clients maintain independence, wellbeing, and connection with their community.

About the Role

- Join a dynamic team of Care Partners with diverse professional backgrounds
- Provide high-quality, person-centred care for a caseload of clients in line with the Aged Care Quality Standards
- Work from our Cheltenham office, with flexibility to visit clients across the Melbourne Metropolitan area, as well as Geelong, the Bellarine Peninsula, and the Mornington Peninsula.
- Build and maintain care plans, monitor client wellbeing, and liaise with internal and external stakeholders to ensure the best outcomes

Key Duties and Responsibilities

- Provide end-to-end support for clients receiving Support at Home Packages, including assessment and commencement




- Work with clients to identify strengths and abilities and deliver appropriate care and services
- Develop, review, and update care plans to support client goals
- Liaise with medical, allied health, and community stakeholders
- Maintain regular client contact, including monthly monitoring calls
- Implement and manage client budgets
- Ensure compliance with Aged Care Quality Standards

Skills and Experience

- Qualifications or experience in Nursing is preferred
- Experience in Case Management is preferred
- Robust time management and organisational skills
- Excellent verbal and written communication
- Ability to build positive relationships with clients, families, and advocates
- Ability to work autonomously and in a team environment
- Current Police Check, driver's licence, and reliable vehicle
- Speaking a second language is an advantage
