As they continue to grow, they are now looking to welcome an experienced Care Coordinator - Registered Nurse into their Brisbane team to support the continued delivery of high quality, clinically informed and participant centred services.

Job Role and Summary:

As the Care Coordinator – Registered Nurse you will report directly to the Director of Operations. This role steps beyond a traditional Nursing role, you will use your clinical expertise to make an impact across the entire participant journey. Managing an allocated portfolio,



you will work closely with participants, families, support workers, Support Coordinators and Healthcare professionals to ensure services are safe, responsive and genuinely person centred. The role combines care coordination, clinical oversight, workforce coordination, compliance, incident management and stakeholder engagement.

Your nursing experience will be particularly valuable when coordinating complex and high intensity supports, medication and Health needs, while identifying and responding to clinical risks. Ultimately, you will help participants receive reliable, high quality care while supporting greater choice, independence and control.

Responsibilities:

- Client assessment, developing tailored care plans, reviews and case management
- Managing budgets and coordinating care services to enhance the client’s quality of life and maintain their independence
- Coordinate participant onboarding, reviews, transitions and ongoing supports
- Understand participant goals, Health needs, risks, preferences and funding arrangements
- Coordinate support workers, rostering, training and participant specific competency requirements
- Oversee service delivery, including SIL supports where applicable
- Coordinate health, medication and high intensity supports with healthcare professionals
- Identify and escalate clinical, medication, safeguarding and service delivery risks
- Manage incidents, complaints, Behaviour Support Plans and relevant NDIS compliance requirements
- Participate in the paid rotating on-call roster and respond to after-hours calls within required response time
- Build strong relationships with participants, families, Support Coordinators, clinicians and allied health professionals
- Maintain accurate participant records, risk assessments and compliance documentation
- Provide day to day guidance to support workers and Team Leaders





Required Skills & Experience:

- Current AHPRA registration as a Registered Nurse is essential
- 2 to 3 years experience within the NDIS and disability sector or Aged Care
- Experience across Support at Home, Home Care and/or DVA programs would be highly regarded
- Strong understanding of clinical risk, medication management and escalation processes
- Experience supporting participants with complex or high intensity care needs
- Working knowledge of NDIS requirements, Practice Standards, safeguarding and behaviour support
- Strong service coordination skills with experience supporting a dispersed workforce
- Excellent communication and problem solving skills, with the ability to build strong stakeholder relationships
- Solid organisation, documentation and reporting skills with the ability to manage competing priorities
- Current NDIS Worker Screening, relevant Working with Children clearance, First Aid and CPR
- Current unrestricted driver's licence, reliable insured vehicle,
- Full working rights in Australia are essential
