Central Adelaide Local Health Network (CALHN) is seeking a Nurse Consultant for Care of Older Persons to lead and coordinate person-centred care across teams, guiding complex discharge planning and improving safety outcomes to avoid unnecessary hospital stays.
You will lead expert nursing care for older people, develop care plans with the team, and strengthen links with residential and community providers while promoting staff development and evidence-based practice.
