In this role, you will deliver high-quality, evidence-based care to patients requiring haemodialysis and hemodiafiltration, supporting them throughout their treatment journey.
Key responsibilities
You will work within a multidisciplinary team to assess, plan and coordinate care tailored to individual patient needs, while engaging respectfully with people from diverse cultural backgrounds.
Provide secure, evidence-based nursing care to renal patients, including assessment, planning, and coordination of care.
Deliver haemodialysis and haemodiafiltration treatments while monitoring patient wellbeing and responding to clinical needs.
Collaborate with multidisciplinary teams and external services to ensure holistic, coordinated care.
