This is a permanent part-time position, offering flexibility while providing the opportunity to make a meaningful difference in the lives of our clients.

As a Community Registered Nurse with Companion Home Care, you will provide clinical nursing services within the client's home, supporting their health, wellbeing and independence. You will work closely with our care team to ensure clinical needs are identified, managed and reviewed in line with each client's care plan.

Your responsibilities may include:

- Conducting comprehensive nursing assessments and identifying changes in clients' health and wellbeing.

- Developing, implementing and reviewing clinical care plans.

- Medication management and medication support.

- Simple and complex wound care.

- Continence assessments and catheter management.

- Diabetes and chronic disease management.

- Stoma care and complex bowel management.

- Supporting clients with complex health conditions, including epilepsy and other neurological conditions.

- Palliative care and end-of-life support.

- Supporting clients following hospital discharge and assisting with their transition back to the home environment.

- Providing clinical advice and support to our care team.

- Completing staff training, education and competency assessments where required.

- Maintaining accurate and timely clinical documentation.

- Communicating effectively with clients, families, GPs, allied health professionals and other relevant healthcare providers.

- Working collaboratively with the broader Companion Home Care team to deliver safe, high-quality, person-centred care.





- Performing other clinical duties reasonably required within the scope of the role.

About You

We are looking for a Registered Nurse who is passionate about community nursing and supporting people to remain independent in their own homes.

You will ideally have:

- Current AHPRA registration with no restrictions or conditions.

- Previous Registered Nurse experience, preferably within community, aged care, home care or a similar setting.

- Experience managing clients with complex health and clinical needs.

- Strong clinical assessment and care planning skills.

- Excellent communication and interpersonal skills.

- The ability to work independently and confidently in clients' homes.

- The ability to work collaboratively as part of a multidisciplinary team.

- Strong organisational and documentation skills.

- A professional, compassionate and person-centred approach.

- A genuine commitment to supporting clients to achieve their health and wellbeing goals.

- The flexibility and willingness to travel between client locations across the Perth metropolitan area.

Essential Requirements

To be considered for this position, you will be required to provide:

- Current AHPRA registration with no restrictions or conditions.

- Current National Police Check, issued within the last six months.

- Full Australian working rights.

- Full WA Driver's Licence.

- Access to a reliable and appropriately insured vehicle.

- Current First Aid and CPR/BLS certification.

- Current Manual Handling certificate.

- Evidence of relevant qualifications and professional development.
