About the Role

n

As the Clinical Care Coordinator, you will provide day-to-day clinical leadership across the home and play a critical role in supporting quality care delivery.

n

Working alongside the Home Manager, you will guide and support our nursing and care teams while ensuring compliance, clinical governance, and best-practice care standards are consistently maintained.

n

Key Responsibilities

n

- Provide hands-on clinical leadership to RNs, ENs and Personal Care Workers
n
- Support clinical decision-making and escalation of resident care needs
n
- Oversee assessments, care planning and clinical documentation
n
- Ensure AN-ACC requirements and documentation standards are met
n
- Support medication management and SIRS incident processes
n
- Monitor clinical indicators, risks, trends and resident outcomes
n
- Lead clinical audits, governance activities and continuous improvement initiatives
n
- Coach, mentor and develop clinical and care staff
n
- Build strong relationships with residents,



families, GPs and allied health professionals
n
- Partner with the Home Manager to achieve clinical and operational objectives
n

About You

n

You are an experienced Registered Nurse who combines strong clinical capability with a genuine passion for leadership.

n

You bring confidence, sound judgement and the ability to influence others while maintaining a highly resident-focused approach.

n

To Be Successful You Will Have:

n

n
- Current AHPRA registration as a Registered Nurse (Division 1)
n
- Minimum 5 years' experience in residential aged care
n
- Previous experience as a Clinical Care Coordinator, Care Manager, Clinical Manager or senior RN
n
- Strong understanding of aged care legislation, accreditation standards and clinical governance
n
- Demonstrated knowledge of AN-ACC and clinical funding requirements
n
- Experience with care planning, assessments and incident management
n
- Exceptional communication and relationship-building skills
n
- Proven ability to lead, coach and develop multidisciplinary teams
n
- A practical, proactive and solutions-focused mindset
n
