Job responsibilities:

- Work 40 hours per week with 8-hour shifts.

- Support care coordination activities across providers to promote continuity of care.

- Educate patients on treatment plans, follow-up instructions, and self-management strategies.

- Use infection prevention practices during patient encounters and procedures.
n
- Perform medication reconciliation activities as directed and communicate discrepancies.
