Horizon Health Care Partners Australia (HHCPA) is a growing Australian telehealth clinic focused on making quality healthcare more accessible, convenient and patient-centred.

We are seeking experienced, patient-focused General Practitioners who would like to join our expanding clinical team and provide care remotely across a diverse range of health services.

This opportunity is particularly suited to GPs and Nurse Practitioners looking for additional income alongside their current employment, flexible sessions from home, or an opportunity to build a larger telehealth workload as HHCPA continues to grow.

A diverse scope of telehealth medicine

Rather than limiting practitioners to one narrow area of care, HHCPA provides patients with access to a broad range of healthcare services.

Depending on your experience, interests and scope of practice, consultations may include:





- General Practice & Primary Care - general medical consultations, acute presentations, ongoing care and preventative health.
- Prescriptions & Medication Management - new and repeat prescriptions where clinically appropriate following consultation.
- Mental Health - assessment, management and ongoing support for common mental health presentations.
- Men's & Women's Health - sexual health, reproductive health, menopause support and other gender-specific healthcare needs.
- Weight & Metabolic Health - medically supervised weight-management and metabolic-health consultations.
- Holistic & Alternative Healthcare - assessment of patients with chronic or complex symptoms and consideration of appropriate treatment pathways within the practitioner's scope and Australian regulatory requirements. Delivering telehealth consultations in line with clinical and regulatory standards
- Assessing patient eligibility and determining appropriate treatment pathways
- Prescribing and reviewing medicinal cannabis therapies where clinically indicated
- Providing follow-up care, treatment adjustments, and patient education
- Working alongside clinical and operational teams to ensure continuity of care
- Keeping accurate, compliant medical records at all times
- Remaining informed on evolving guidelines, legislation, and clinical evidence
- Health Optimisation & Longevity - preventative health, metabolic optimisation, healthy ageing, performance, recovery and evidence-informed health optimisation.
- Chronic Disease & Preventative Care - ongoing management and continuity of care for suitable telehealth patients.
- Pathology & Radiology - assessment and clinically appropriate investigation/referral.
- Medical Certificates & General Telehealth Services - convenient access to appropriate everyday healthcare.





Who we're looking for

We would love to hear from GPs and Nurse Practitioners who are comfortable working independently in a telehealth setting while maintaining high standards of clinical governance and patient care.

You will ideally have:

- Current Unrestricted AHPRA registration as a Medical Practitioner
- Appropriate qualifications and eligibility to practice independently in Australia
- Current professional indemnity insurance
- Excellent communication and clinical decision-making skills
- Confidence conducting real-time telehealth consultations
- A patient-centred and evidence-informed approach
- Strong documentation and prescribing practices
- An understanding of relevant Australian prescribing and telehealth requirements
- Interest in one or more of HHCPA's clinical service areas
- Willingness to work collaboratively with our administration and clinical team
- weight management, women's or men's health, mental health, chronic pain, holistic medicine, health optimisation or alternative therapies is highly regarded but not essential.

You don't necessarily need to leave your existing practice or employment to join HHCPA.

Start with a few hours or sessions each week, nominate the times that work around your existing commitments and increase your availability as your HHCPA patient base develops.

Remuneration

60-70% of practitioner billings

Contractor arrangement with flexible availability.

Your earning capacity will depend on consultation mix, appointment volume, consultation length and your individual availability.
