Illawarra Shoalhaven Local Health District in North Wollongong is seeking an Aboriginal Health Worker to support the Out of Home Care Health Pathway Program. You will coordinate health assessments, assist in Health Management Plans, and liaise with NGOs to improve outcomes for Aboriginal children and young people.

You will be the primary contact for carers and caseworkers, ensuring culturally protected and trauma-informed care within a multidisciplinary team, with a temporary appointment to 30 June
