Calvary Health Care is seeking a Clinical Project Lead to drive the Dementia Model of Care across hospital, residential aged care and home care services. You will collaborate with consumers, families, clinicians and operational leaders to deliver person-centred, evidence-informed dementia care and oversee implementation activities.

You will partner with service leaders to assess readiness, mitigate barriers, and sustain improvements through education, governance, and practical tools.
