In this role, you will deliver high-quality, evidence-based care to patients requiring haemodialysis and hemodiafiltration, supporting them throughout their treatment journey.
Key responsibilities
- You will work within a multidisciplinary team to assess, plan and coordinate care tailored to individual patient needs, while engaging respectfully with people from diverse cultural backgrounds.
- Provide protected, evidence-based nursing care to renal patients, including assessment, planning, and coordination of care.
- Deliver haemodialysis and haemodiafiltration treatments while monitoring patient wellbeing and responding to clinical needs.
- Collaborate with multidisciplinary teams and external services to ensure holistic, coordinated care.
