The position functions as part of a multi-disciplinary team with a teamwork approach to the provision of low intensity therapy home-based programs to frail elderly clients who are ready for discharge from sub-acute hospital care but are unable to return home without additional care and support.
Key responsibilities
- As the Physiotherapist in ITACS you will provide high quality Physiotherapy services to clients and carers, to ensure the provision of optimal health outcomes for the clients.
- You will be required to have problem-solving skills to support a client’s secure mobility in the home and community setting without direct clinical supervision when completing in-home assessments and interventions.
- You will also provide care planning, case management and liaise with external stakeholders.
