BLUE APPLE SUPPORT SERVICE PTY LTD The Aged or Disabled Care Worker will provide high-quality, person-centred care and support to elderly individuals and people living with disabilities, assisting them to maintain independence, dignity, and wellbeing within their homes and community settings. The successful candidate will assist with daily living activities, personal care, community participation, and emotional support while ensuring services are delivered in accordance with individual care plans, organisational policies, and relevant legislation.

Task & responsibilities Provide person-centred care and support in accordance with individual care plans. Assist clients with personal care, including bathing, dressing, grooming, toileting, and personal hygiene.

Support clients with mobility, transfers, positioning, and the protected use of mobility aids. Assist with meal planning, food preparation, feeding, and monitoring nutritional requirements.



Support clients with medication assistance in accordance with organisational policies and relevant legislation.

Perform household tasks including cleaning, laundry, shopping, and light domestic duties. Assist clients to participate in social, recreational, educational, and community activities to promote independence and inclusion. Provide emotional support, companionship, and encouragement to enhance clients' wellbeing.

Observe, monitor, and report changes in clients' health, behaviour, or care needs to supervisors. Maintain accurate care records, progress notes, and incident reports. Follow infection prevention, manual handling, workplace health and safety (WHS), and risk management procedures.

Communicate effectively with clients, families, healthcare professionals, and multidisciplinary teams. Promote clients' rights, privacy, dignity, cultural diversity, and independence at all times. Maintain confidentiality and comply with NDIS Practice Standards, organisational policies, and relevant legislation.

Qualification & experience AQF Certificate III or Certificate IV in Individual Support (Ageing and/or Disability), Aged Care, Disability, Community Services, or an equivalent recognised qualification. Minimum 1-3 year of relevant full-time experience providing aged care,



disability support, community care, or home care services. Demonstrated ability to provide person-centred support to individuals with varying physical, intellectual, or psychosocial support needs.

Good understanding of safe manual handling, infection control, duty of care, and workplace health and safety requirements. Excellent communication, interpersonal, and relationship-building skills. Ability to work independently and as part of a multidisciplinary team.

Strong organisational and time management skills with accurate record-keeping abilities. Willingness and ability to work a minimum of 38 ordinary hours per week, with reasonable additional hours as required by operational needs.
