Full-time (Flexible Location- based within a Country and Outback Health office)
About The Role
As a NDIS Support Coordinator, you will work closely with participants to maximise the usage of their plans to support building independence and progressing towards their goals. You will be pivotal to linking participants with appropriate services and providing high-quality person-centred support. A component of this role may include our CPSP Program which includes assisting clients to identify and progress towards recovery goals and linking clients with relevant local services and supports.
About You
If you are passionate about empowering people to achieve their goal and live independently and have the following:
- Previous experience in NDIS Support Coordination will be highly regarded.
- Experience working in the disability, community or mental health sector.
- Compassionate, resilient and a natural problem solver
- Confidence working independently while also contributing to a supportive team environment.
- Strong organisational and time-management skills and the ability to manage a caseload independently.
- Excellent communication skills and ability to build meaningful relationships.
- Proficient in MS Office and previous experience using CRM platforms.
