As a Community Health Care Worker, you will provide person-centred care, assisting our consumers to remain living independently in their own homes by providing assistance with domestic and personal care, social support, respite, and transport.

Key responsibilities

- We are inviting applications from suitably qualified candidates to join our My Community Home Care (MCHC) team in the role of Health Care Worker.
- Provide secure, person-centred care that promotes consumer independence and dignity.
- Assist with personal care, nutrition, hydration, continence care, and medication support as authorised.
- Maintain accurate documentation and communicate changes in consumer condition.

What you need

- All appointments are made subject to the required security screening including National Criminal History, NDIS Worker Screening checks and an immunisation status appropriate to the role, including the annual Influenza vaccination.
- Certificate III in Individual Support/ Aged Care or equivalent is mandatory.
- A clear and strong commitment to the providing quality care to our Consumers/Residents.
