About the role

HFH CARE PTY LTD is seeking experienced and reliable Support Workers to provide ongoing support to a male participant in his 60s with complex, high-support needs. The participant has a brain injury, cerebral palsy, and one amputated leg. He is able to walk with assistance and also uses a wheelchair. He lives at home with his wife and requires support with personal care, mobility, community access, meal preparation, toileting, and overnight assistance.

Key responsibilities
Provide showering and full morning personal care
Assist with dressing and hygiene support
Prepare breakfast and dinner
Provide toileting and continence assistance
Assist with mobility and transfers
Support community access and transport to medical and other appointments
Provide assistance with daily activities around the home
Provide active overnight support and remain awake and available throughout night shifts to respond to the participant's needs, including assistance with transfers and toileting approximately 4–5 times throughout the night

About you
Previous experience supporting participants with complex or high physical support needs
Experience with personal care, continence support, transfers, mobility assistance, and wheelchair support
A nursing, disability, aged care, or similar healthcare background is highly regarded
Solid spoken English and excellent communication skills
Confidence providing respectful and dignified personal care
Reliability, patience, professionalism, and a calm approach
Ability to work independently and follow established care routines
Availability for regular and ongoing shifts
All relevant checks, qualifications, and requirements applicable to disability support work, including appropriate screening, First Aid/CPR, and a valid driver licence where transport duties are required
