Join the dedicated team at Trinity Adventure Projects as a Support Coordinator, providing essential support and guidance to young people and their families in relation to their NDIS services. In this full time role, you will play a pivotal part in empowering vulnerable individuals and families, helping them navigate challenging circumstances and achieve positive, sustainable outcomes.
Key responsibilities
Manage a caseload of young people and their families, conducting comprehensive disability needs assessments and developing tailored support plans
Coordinate a range of specialised disability services, including referrals to partner organisations, and practical assistance
Collaborate closely with other professionals, such as social workers and youth workers, to ensure holistic, wrap-around care
Advocate for clients' rights and ensure they have access to appropriate resources and opportunities
Maintain detailed case notes and records, contributing to the continuous improvement of service delivery
Participate in team meetings and professional development activities to enhance your skills and knowledge
About you
Tertiary qualification in human services, criminology, social work, psychology, or a related field
Minimum 2 years' experience working in a similar case management, youth/family support role or support coordination role
Demonstrated understanding of the challenges facing young people and families in vulnerable situations
Knowledge of NDIS processes, Support Coordination principles, and disability service frameworks
Excellent communication and interpersonal skills, with the ability to build trust and rapport with diverse clients
Proven ability to work collaboratively within a multidisciplinary team
Solid organisational skills and attention to detail
