About the role
You will work as part of an integrated multidisciplinary Allied Health team delivering person-centred services across aged care, restorative and community settings.
Key responsibilities
Mobility & transfer assessment
Balance & falls risk assessments
Multiple outcome measure administration i.e DEMMI, SPPB for tracking functional progress
Individualised rehabilitation with graded loading, falls-prevention strategies and group exercise class facilitation
Post falls review
Working collaboratively in a multidisciplinary team, including attending multidisciplinary case conferences alongside doctors, pharmacists and allied health staff, contributing to discharge planning, care pathways and clinical decision-making
Manual handling assessments and care staff education
Reablement and restorative interventions
About you
Work experience in community aged care
Experience working with TCP clients is desirable but not essential
Passionate about older people and quality care
Practical, collaborative and compassionate
Strong communicators
Open to innovation and service improvement
Registered with AHPRA
