About the Role

As a Home and Community Registered Nurse, you will provide compassionate, person-centred care tailored to each client’s unique needs. You’ll support their well-being and independence through clinical care, health education, and collaboration with families and other healthcare professionals.

Key Responsibilities
Health Monitoring and Care: Blood pressure, blood sugar, pain, and medication management.
Specialised Care: Palliative care, dementia support, continence management, and wound care.
Client Education: Support clients and families in managing conditions like diabetes and maintaining healthy lifestyles.
Falls Prevention and Safety: Implement falls risk strategies and promote secure mobility.
Collaboration and Coordination: Work with allied health professionals and provide carer education to ensure holistic care.
Disability Support: Manage behaviour support,



assistive technology, and Specialist Disability Accommodation (SDA).

What We’re Looking For We are seeking compassionate and adaptable Registered Nurses with excellent communication skills and a proactive approach to client care.

What You’ll Need
Current AHPRA registration as a Registered Nurse
First Aid and CPR certification
Minimum 3 years of experience in home or community nursing
Valid driver’s licence and reliable vehicle
National Police Check and Working with Children Check (if applicable)

Desirable
Postgraduate qualifications in community health, aged care, or mental health
Familiarity with NDIS guidelines and disability care
