Case Manager - Community Aged Care
Step into a stable Case Manager position with a trusted Brisbane based not-for-profit. Manage a capped caseload and enjoy the backing of a supportive, values-driven team.
n As Case Manager, you’ll work closely with Support at Home clients building robust relationships, tailoring care plans, and ensuring each person receives the right services to support their wellbeing and independence.

The Role n
n

n
Conduct in-home assessments, reviews, and personalised care planning
n
Coordinate services and manage referrals/onboarding of new clients
n
Oversee Support at Home budgets in line with client goals and priorities
n




Collaborate with clinical and support staff to deliver quality outcomes
n
Monitor changes in client circumstances and adjust care accordingly
n
Ensure compliance with Aged Care Quality Standards and organisational policies
n

What We’re Looking For n

n
1–2 years’ experience in community aged care
n
Excellent communication and relationship-building skills
n
Strong organisational skills with the ability to manage priorities
n
A proactive, resilient mindset with a client-first approach
n
Knowledge of aged care compliance and quality standards
n
Current Australian driver’s licence
n
