We are seeking a dedicated Enrolled Nurse / Aboriginal Health Practitioner to join our Primary Care Services. You will work with the team to deliver high-quality, culturally safe care, support clients with health education and chronic condition management, and build positive relationships with clients and stakeholders. Your role will also involve maintaining confidentiality, contributing to a positive team setting, and supporting our organisational standards and accreditation.

About the Role
n
You will:
n
n
Work collaboratively with all members of Primary Care Services to provide day-to-day client care
n
Work collaboratively to ensure the delivery of high quality and culturally safe client care and contribute to the smooth running of the clinic
n
Liaise with and broker services between clients and internal and external stakeholders as necessary
n
Provide health education and services to clients on issues of some complexity, including chronic conditions self-management
n
Perform tasks of a sensitive nature including the provision of health information requiring the exercise of judgement, initiative and confidentiality.
n
Maintain a positive and harmonious working relationship and actively contribute to organisational activities
n
Participate in personal and professional development activities
n
Ensure compliance with a range of administrative practices which support Primary Care Services and Nunkuwarrin Yunti as an Organisation, including accreditation.

About You
n
You bring:
n
n
Complete qualification in Diploma of Nursing or Cert IV
n
Current AHPRA Registration as an Enrolled Nurse (Division 2) or AHP)
n
AHPRA registered AHP or completed Cert IV in Aboriginal and/or Torres Strait Islander Primary Health Care Practice
n
Demonstrated understanding of the contemporary issues facing Aboriginal people and the impacts these issues have on Indigenous Australian societies and cultures and health
n
Experience using computer software, especially Patient Information Recall Systems and other databases.
n
Well-developed clinical skills and a sound knowledge of best practice approaches to comprehensive primary health care.
n
Organisational skills, self-confidence and the ability to work independently and autonomously, assess priorities, organise workload and meet deadlines
n
Ability to resolve conflict, solve problems and negotiate outcomes.
n
Demonstrated ability to communicate sensitively and effectively with Aboriginal and Torres Strait Islander people, organisational staff, and other providers.
n
Ability to communicate effectively both written and verbally, with a wide range of people from Aboriginal communities, government and other agencies, committees and other bodies.
n
Knowledge of and demonstrated commitment to workplace diversity, workplace participation, Work Health and Safety

Skills And Experiences
n
n
n
Desirable:
n
n
Experience working within a Community Controlled Health Service.
n
Significant experience in care planning (including but not limited to, the preparation of wellness plans, asthma and diabetes cycle of care plans).
n
Knowledge of existing social / health issues within the Aboriginal and Torres Strait Islander communities.
n
Knowledge of contemporary Aboriginal culture and societies and aspirations of Aboriginal people.
n
Experience using the Communicare patient recall information system.
n
Experience in continuous quality improvement activities including clinical file auditing
n
