Job Closes Date: 30 Days About the Role Our client delivers person-centred care that empowers individuals with disabilities to live independently and participate fully in their communities.
As a Community Worker, you will support clients in achieving their personal goals, enhancing their quality of life, and fostering social inclusion.
This role involves working closely with families, carers, and local networks to create sustainable support solutions.
Key Duties and Responsibilities • Conduct comprehensive assessments of clients' physical, emotional, and social needs, then design and implement individualized support, training, and educational programs that promote independence and skill development.
• Interview clients and their families to identify underlying challenges, such as housing instability, employment barriers, or social isolation, and develop tailored intervention strategies.
• Regularly monitor client progress, document outcomes, and adjust support plans to ensure goals are met, providing detailed reports to internal teams and external stakeholders.
• Connect clients with specialized agencies—including health services, housing authorities, and vocational trainers—to address complex or additional needs beyond our scope.
• Evaluate community resources and service gaps in areas like health, welfare, housing, and employment, and collaborate with local organizations to develop new initiatives that benefit clients.
• Liaise with community groups, government bodies,
and private businesses to raise awareness of available services, advocate for client needs, and promote inclusive practices.
• Provide direct support to families, including education on disability care, behavioural guidance, and crisis intervention, while ensuring a secure and nurturing environment for children and adults in group homes or day programs.
• Facilitate life-skills workshops, recreational activities, and social integration programs that encourage community participation and peer connection.
• Prepare funding submissions, grant proposals, and progress reports for government agencies and partner organizations to secure ongoing resources.
Essential Requirements • Diploma in Community Services, Social Work, or related field (or equivalent experience).
• 1 year of full-time work experience within community services, social work, or in related positions.
• Strong interpersonal and communication skills, with the ability to build trusting relationships with diverse clients and stakeholders.
• Experience in case management, program delivery, or client advocacy within the disability or community sector.
• Current First Aid/CPR certification and a valid Working with Children Check.
• Ability to work independently and as part of a multidisciplinary team, with a proactive, empathetic approach.
• Relevant qualification to above field.
