We have an exciting opportunity for a registered nurse to join our Clinical and Nursing services team, providing nursing services to the East Gippsland community.
n
The role of the primary health care nurse incorporates a range of activities which contribute to the optimal functioning of the GP Practice and supporting access to quality of primary care services to the smaller communities surrounding Lakes Entrance.
n
The position will work closely with other service providers including general practitioners, allied health, speciality nursing services, and other community support services to ensure a coordinated approach to client care.
n
The scope of the role includes: triage, assessment and initial management of acute presentations; pathology collection; preventative health and early intervention initiatives including education,



screening, coaching & ongoing monitoring of health status; bio-psychosocial assessment of clients with chronic health conditions using a client centred approach incorporating health change methodology; and documenting of shared support/ care plans in collaboration with the interdisciplinary team members including coordination of case conferences or care planning meetings as required.
