About The Role n
Join our friendly, passionate, and deeply supportive Aboriginal Health team dedicated to delivering exceptional, culturally safe care to First Nations patients and their families.
n
n
Deliver holistic social, emotional,
& advocacy support to First Nations patients and their families
n
Conduct culturally safe assessments & drive meaningful engagement to support healthcare decisions
n
Collaborate with multidisciplinary teams, participate in family conferences, and coordinate discharge planning
n
Help foster strong partnerships with local Aboriginal community health providers and external services
n
Serve as a cultural resource for hospital staff while supporting health promotion & improvement initiatives
n
About You n
n
Identify as Aboriginal and/or Torres Strait Islander
n
Experienced in supporting First Nations patients and their families in a healthcare setting
n
Strong understanding of the healthcare challenges facing First Nations patients and families
n
Excellent communication & teamwork skills coupled with the ability to manage competing priorities
n
Have or be willing to undertake a minimum Certificate III in Aboriginal Primary Health Care
n
A current unrestricted NSW Driver's License
n
Experience in analysing patient and service metrics, desirable
n
