The Role
n
Working within Residential Aged Care Homes in Traralgon, Paynesville or both, you will provide advanced assessment, diagnosis, prescribing, treatment and care planning for residents. You will work within a collaborative multidisciplinary model alongside on-site nurses, General Practitioners, telehealth Geriatricians and Physicians, with dedicated administrative and operational support so your focus remains on delivering high-quality clinical care.

Opportunities are available across:

Traralgon – one regular weekday per week, 9AM - 5PM
n
Paynesville – one regular weekday per fortnight, 9AM - 5PM
n
n
Applications are welcomed from Nurse Practitioners interested in either location individually or across both locations.

What You'll Bring
n
n
Experience in aged care, gerontology, palliative care or chronic disease management is highly regarded
n
Advanced clinical experience in assessment, diagnosis, treatment and health promotion
n
Strong clinical reasoning, communication and leadership skills
n
Experience in quality improvement and evidence-based practice
n
Sound knowledge of aged care legislation and regulatory requirements
n
A collaborative, resident-centred approach to care
n




Comfortable using technology and clinical software systems
n
Current driver's licence, reliable vehicle and appropriate professional indemnity insurance
n

Qualifications
n

Master of Nursing (Nurse Practitioner)
n
Current AHPRA registration as a Registered Nurse with Nurse Practitioner endorsement
