About the role Provide advanced clinical leadership to the community services intake function, ensuring timely, equitable and patient-centred access to care.
Working collaboratively with multidisciplinary teams and key stakeholders, you will coordinate referrals, clinical triage and service access while supporting quality outcomes and operational efficiency.
Lead the centralised intake function, coordinating the referral, clinical triage, prioritisation and allocation of referrals to ensure timely access to community-based services.
Manage service demand and waitlists by monitoring referral activity, prioritising care needs and reduce waiting times and optimise service capacity.
Provide expert clinical leadership and decision-making, ensuring referrals are assessed and allocated in accordance with clinical need,



service criteria and best practice standards.
Collaborate with multidisciplinary teams, referrers, general practitioners, hospitals and community providers to facilitate coordinated, seamless and patient-centred care.
Monitor, analyse and report on referral, waitlist and service activity data to identify trends, inform workforce planning and support continuous service improvement.
Practice with a high degree of autonomy and accountability, demonstrating advanced nursing expertise, leadership and delegation in accordance with professional standards and organisational requirements.
