Inglewood District Health Services Community and Home Support Team are seeking an experienced, enthusiastic and capable Care Partner with an interest in rural health, to join our friendly and expanding team as we expand our service into Support at Home.
About Inglewood
Inglewood & Districts Health Service (IDHS) is situated in the Shire of Loddon and provides services to more than 5,500 residents in the communities of Bridgewater, Inglewood, Korong Vale, Newbridge, Serpentine, Tarnagulla, Wedderburn and surrounding townships.
IDHS encompass urgent care, acute, allied health and residential aged care as well as providing extensive outpatient and community services.
IDHS offers a diverse range of community and home care services including allied health, health promotion, chronic disease management, mental health & AOD counselling, social support, home and personal care and strength training programs.

The Position
The Care Partner/ Case Manager is responsible for coordinating and overseeing a high-quality, person-centred Support at Home service for a portfolio of older people living in the community.
The role acts as the key point of contact for clients, their families and representatives, ensuring that services are coordinated around the client's individual goals, needs, preferences and circumstances.
The Care Partner/ Case Manager develops and reviews care and support plans, coordinates services and providers, monitors client wellbeing and outcomes, identifies and manages risks, and advocates for clients to ensure they receive appropriate and timely support.
The role requires a strong understanding of ageing, independence, safeguarding,



community-based care and the principles of consumer choice and control.
About the role:
This role provides case management and care coordination for Support at Home clients, ensuring services align with individual goals, needs and funding.
Responsibilities include conducting assessments, developing and reviewing care plans, coordinating service providers, monitoring wellbeing and service delivery, and facilitating access to health and community supports.
The position also identifies and manages risks, responds to safeguarding concerns, advocates for client choice and independence, and supports clients to navigate aged care and community services.
Working closely with clients, families and stakeholders, the role promotes positive outcomes through person-centred, culturally appropriate care.
In addition, the role monitors budgets and service utilisation, maintains accurate documentation, ensures compliance with legislative and organisational requirements, and contributes to quality improvement initiatives and continuous professional development.
Essential criteria
Minimum tertiary requirement: Diploma or higher qualification in Community Services, Aged Care, Nursing, Health or Human Services, combined with demonstrated case management experience, will be considered,
Minimum of 2 years' experience in the following:
working with Support at Home or previous Home Care Package programs,
coordinating complex or higher-risk clients,
working with clients experiencing cognitive decline, social isolation, complex family circumstances or multiple health and support needs,
Experience working within the Australian aged care system,




Knowledge of My Aged Care processes and community aged care services,
Desirable
Knowledge of the service system and ability to liaise and advocate across the full spectrum of health and community care.
Experience with and a passion for service and quality development
Experience of regional and rural health delivery
