About the Role
As the Clinical Care Coordinator, you will provide day-to-day clinical leadership across the home and play a critical role in supporting quality care delivery.
Working alongside the Home Manager, you will guide and support our nursing and care teams while ensuring compliance, clinical governance, and best-practice care standards are consistently maintained.
Key Responsibilities
Provide hands-on clinical leadership to RNs, ENs and Personal Care Workers
Support clinical decision-making and escalation of resident care needs
Oversee assessments, care planning and clinical documentation
Ensure AN-ACC requirements and documentation standards are met
Support medication management and SIRS incident processes
Monitor clinical indicators, risks, trends and resident outcomes
Lead clinical audits, governance activities and continuous improvement initiatives
Coach, mentor and develop clinical and care staff
Build strong relationships with residents, families,
GPs and allied health professionals
Partner with the Home Manager to achieve clinical and operational objectives
About You
You are an experienced Registered Nurse who combines strong clinical capability with a genuine passion for leadership.
You bring confidence, sound judgement and the ability to influence others while maintaining a highly resident-focused approach.
To Be Successful You Will Have:
Current AHPRA registration as a Registered Nurse (Division 1)
Minimum 5 years' experience in residential aged care
Previous experience as a Clinical Care Coordinator, Care Manager, Clinical Manager or senior RN
Strong understanding of aged care legislation, accreditation standards and clinical governance
Demonstrated knowledge of AN-ACC and clinical funding requirements
Experience with care planning, assessments and incident management
Exceptional communication and relationship-building skills
Proven ability to lead, coach and develop multidisciplinary teams
A practical, proactive and solutions-focused mindset
