Worker/Practitioner
to join its multidisciplinary primary health care team in Queensland. The role delivers culturally protected, person-centred primary health care and supports clients to access services and engage with care, working alongside GPs, nurses and allied health staff.

Key duties include health assessments, chronic disease management, care planning and health promotion while maintaining medical records.
