Ozcare is seeking an experienced and passionate Care Partnering Lead to provide leadership and support to our Care Partner team servicing the Gladstone region. This permanent part-time opportunity offers the chance to play a key role in driving service excellence, supporting team development, and contributing to the growth of our Home Care Package services while working closely with a remote team based in Gladstone.Your RoleReporting to the Clinical & Care Partnering Manager, you will provide leadership, coaching and operational support to a team of Care Partners, ensuring the delivery of high-quality, client-focused care coordination services. You will play an integral role in supporting service growth, maintaining compliance, driving continuous improvement initiatives, and fostering strong relationships with clients, families, healthcare professionals and community stakeholders.As a Care Partnering Lead you will:Based at our Rockhampton Branch you will lead, coach and support a remote Care Partner team servicing the Gladstone regionEnsure care coordination services are delivered effectively, efficiently and in line with organisational standards and client expectationsSupport Care Partners to assess, plan, coordinate and review services that promote client independence, wellbeing and choiceProvide guidance and oversight in the development and management of Home Care Package budgets, quotes and care plansFoster strong relationships with clients, families, referrers and key stakeholders to support positive client outcomesMonitor service quality, customer feedback and performance outcomes, implementing improvement initiatives where requiredProvide supervision,



performance support and professional development opportunities for direct reportsLead problem-solving and escalation management relating to client services and care deliverySupport compliance with aged care legislation, quality standards, organisational policies and continuous improvement requirementsMonitor reporting, documentation, billing processes and data integrity to ensure accuracy and complianceSupport local service growth initiatives through networking, stakeholder engagement and community promotionWork collaboratively with multidisciplinary teams to ensure seamless and coordinated service deliveryHow You'll BenefitYou'll become part of the Ozcare family, and in addition to the training and support that you would expect, you will also receive:$56.60 per hour + Super + NFP Salary Packaging Benefits5 weeks annual leave + 14% annual leave loadingOptional salary packaging, meal entertainment cards and novated leasesPrivate health insurance discountsUniform and laundry allowanceOngoing professional development and leadership supportThe opportunity to make a meaningful impact in the lives of older Australians and their familiesRole Requirements:Demonstrated experience in community aged care, home care packages, care coordination, case management or a related health/community services environmentProven leadership experience with the ability to coach, mentor and support team membersStrong knowledge of Home Care Packages,



client-centred care and service delivery frameworksExperience managing client budgets, service plans and compliance requirementsHighly developed verbal, written and interpersonal communication skills with the ability to engage a broad range of stakeholdersDemonstrated ability to build solid relationships and collaboratively work across multidisciplinary teamsStrong organisational skills with the ability to manage competing priorities and meet deadlinesTertiary qualifications and/or relevant experience in a health, aged care or community services related discipline (e.g. Health and Human Services)Intermediate computer skills including Microsoft Office applications and client management systemsCurrent First Aid and CPR CertificateCurrent National Police Check (valid within the past 3 years) or a willingness to obtain oneCurrent Queensland Driver LicenceProof of immunity, or willingness to become vaccinated against, Measles, Mumps, Rubella (MMR), Varicella (Chicken Pox) and Pertussis (Whooping Cough)To be considered, candidates must possess the availability to work a recurring roster of Monday to Friday (8:30am - 3:00pm)Welcoming AllWe welcome everyone to become part of the Ozcare family regardless of belief, ethnic or social background, health and gender.
