This permanent part-time opportunity offers the chance to play a key role in driving service excellence, supporting team development, and contributing to the growth of our Home Care Package services while working closely with a remote team based in Gladstone.Your RoleReporting to the Clinical & Care Partnering Manager, you will provide leadership, coaching and operational support to a team of Care Partners, ensuring the delivery of high-quality, client-focused care coordination services. You will play an integral role in supporting service growth, maintaining compliance, driving continuous improvement initiatives, and fostering strong relationships with clients, families, healthcare professionals and community stakeholders.As a Care Partnering Lead you will:Based at our Rockhampton Branch you will lead, coach and support a remote Care Partner team servicing the Gladstone regionEnsure care coordination services are delivered effectively, efficiently and in line with organisational standards and client expectationsSupport Care Partners to assess, plan, coordinate and review services that promote client independence, wellbeing and choiceProvide guidance and oversight in the development and management of Home Care Package budgets, quotes and care plansFoster strong relationships with clients, families, referrers and key stakeholders to support positive client outcomesMonitor service quality, customer feedback and performance outcomes, implementing improvement initiatives where requiredProvide supervision,
