About Us
YH Support Services is a registered NDIS service provider operating in Elizabeth and the northern regions of Adelaide. We provide person-centred disability support, including SIL/SDA accommodation, personal care and community support, helping participants live independently and achieve their individual goals.
Duties & Responsibilities
Assist participants with personal care, including showering, dressing, toileting and personal hygiene.
Support participants with mobility, transfers and daily living activities.
Prepare meals and assist with food and nutrition requirements.
Provide domestic assistance, including cleaning, laundry, shopping and household duties.
Support participants with community access, social and recreational activities.
Provide transport to appointments and community activities.
Provide companionship, emotional support and reassurance.
Assist participants to develop independent living skills and achieve personal goals.
Support participants in accordance with individual care and support plans.
Monitor participants' wellbeing and report changes or concerns.
Maintain participant records and progress notes.
Qualifications & Experience
Certificate III in Disability or equivalent, or relevant work experience.
At least one year of relevant work experience.
Current First Aid and CPR.
NDIS Worker Screening Clearance.
Working with Children Check and Police Check.
Medication Administration Certificate, or willingness to obtain.
Valid driver's licence and comprehensive vehicle insurance.
