About the role This is an exciting opportunity to join a dynamic Program Coordination-Aged Care Team. The focus of the role is to ensure that the care delivered to clients is planned, goal focused and is individually tailored for each recipient. The role is demanding and requires the successful candidate to possess a high level of interpersonal, communication, mediation and conflict resolution skills with the ability to interact with a range of professional people, organisations and consumers.

You will also have the ability to display understanding, empathy and have the capacity to work harmoniously within a busy team environment.

About you

- Qualification in a relevant discipline or currently working towards same
- Significant experience within the Aged Care sector
- Preference for candidates with previous experience in Support at Home Package Management
- Working With Vulnerable People registration (Employment) with NDIS Worker Screening endorsement
- Satisfactory National Police Check
