About the Role

We're looking for a passionate Care Partner to join our team as a relationship-based care manager. As the designated care manager under the Support at Home Program, you'll coordinate tailored, responsive support for participants — working closely with clinically qualified Care Partners, allied health professionals, and interdisciplinary teams to keep older people living safely and independently at home.

Key Responsibilities

- Develop individualised care plans in collaboration with participants, aligned to their goals, cultural needs and approved Support Plan
- Establish and manage quarterly budgets, ensuring participants understand their services and financial responsibilities
- Coordinate care across the Restorative Care Pathway, End-of-Life Pathway, and Assistive Technology & Home Modifications (AT-HM), escalating to clinically qualified Care Partners where needed




- Conduct regular care reviews and proactively manage emerging risks or changes in participant needs
- Build strong relationships with participants, supporters, GPs, allied health and service coordinators
- Promote participant choice, autonomy and supported decision-making
- Maintain accurate, timely documentation in line with the Strengthened Aged Care Quality Standards

About You

- A natural organiser who thrives on juggling priorities, with sharp attention to detail that keeps care plans and budgets on track
- Calm, confident and solutions-focused — even when the pressure's on
- A skilled communicator who builds trust easily, whether on the phone, in a home visit, or in writing
- A true team player who knows outstanding care outcomes come from strong collaboration with clinicians, allied health and families
- Current Police Check and First Aid certification
- Confident using Microsoft Word and Excel to manage documentation and budgets accurately
