What We Are Looking For Chosen Family is seeking compassionate, reliable and inclusive Support Workers to join our growing network for participant-matching opportunities across Sydney CBD and surrounding suburbs.

We work with people from diverse backgrounds and with a broad range of support needs. Depending on the participant and opportunity, our Support Workers may provide assistance across disability support, aged care, mental health, community access, daily living, social support, childcare and LGBTQIA+ support.

We welcome Support Workers of all gender identities, including female-identifying, male-identifying, transgender, non-binary and gender-diverse applicants.

We particularly value people who are LGBTQIA+-affirming, culturally aware, trauma-informed and respectful of individual identity, communication styles, preferences and lived experiences. Lived experience as a member of the LGBTQIA+ community is highly regarded.

You don't need to be the right person for every participant. Our focus is on finding the right match between each participant and Support Worker based on experience, skills, personality, availability, location, and individual preferences.

Responsibilities Of The Role Depending on the participant and their individual support plan, responsibilities may include:

- Supporting people to live independently and participate meaningfully in their community.
- Providing person-centred disability and community support tailored to individual goals and preferences.
- Supporting older people with companionship, daily living and community participation.
- Providing respectful support to people experiencing mental health challenges, using a calm, trauma-informed and recovery-oriented approach.
- Supporting children and young people with age-appropriate activities, routines, community access and social participation where relevant to the role.
- Assisting with household tasks, meal preparation,



personal care and daily living activities where required.
- Supporting participants to attend appointments, activities and community events.
- Providing companionship, social connection and emotional support.
- Building positive and trusting relationships with people from diverse backgrounds, cultures, gender identities and life experiences.
- Using positive behaviour strategies and adapting communication to each person's needs.
- Supporting participants to develop confidence, independence and practical life skills.
- Respecting individual boundaries, choices, routines, identities and preferences.
- Maintaining accurate progress notes and completing required documentation.
- Demonstrating clear, professional written and verbal communication.
- Working collaboratively with participants, families, Support Coordinators and the Chosen Family team.
- Providing dependable and consistent support and communicating promptly regarding availability or changes.

Applicants Must Have

- NDIS Worker Screening Check
- Working with Children Check (WWCC)
- First Aid Certificate
- Valid Australian Driver's Licence
- Access to a reliable vehicle
- Ability and willingness to travel for up to approximately 30 minutes to support participants
- Availability for shifts of approximately 3–4 hours or longer, depending on participant requirements
- A commitment to respectful, person-centred and inclusive support
- LGBTQIA+ awareness and a willingness to support people of diverse gender identities and sexual orientations respectfully

Additional qualifications, experience and certifications relevant to disability, aged care, mental health, childcare or community services are highly regarded. Why Join the Chosen Family Network?
