Kurri Kurri Community Services (KKCS) is local not-for-profit organisation delivering person centred services across the Hunter.

Hunter Recruitment Group is partnering with KKCS to recruit an Enrolled Nurse Care Partner to join their Support at Home team.

This is a great opportunity for an Enrolled Nurse who enjoys building relationships, coordinating holistic care and working within the community, while continuing to utilise their clinical skills as part of a supportive multidisciplinary team.

About the Role

KKCS has recently introduced a collaborative pod model within their Support at Home team.

As an Enrolled Nurse Care Partner, you'll work closely with a Non-Clinician Care Partner, supported by Registered Nurse clinical oversight, to coordinate care for a shared group of older people living within the community.

You'll be responsible for the clinical aspects of care, including care planning, reviews, assessments and identifying changing care needs, while working closely with your Care Partner to ensure services, supports and funding continue to align with each consumer's individual needs and goals.

This is a varied community based position, combining care coordination with regular home visits and the chance to maintain your clinical skills through appropriate nursing care within your scope and competencies.

What You'll Be Doing

- Complete clinical assessments, care planning and ongoing consumer reviews
- Conduct home visits and build trusted relationships with consumers and families
- Identify changing clinical needs and coordinate appropriate responses




- Support onboarding and ongoing reviews
- Provide appropriate clinical care within your EN scope and demonstrated competencies
- Complete wound care and catheter care where trained and competent
- Maintain accurate clinical documentation and consumer records
- Liaise with families, carers, healthcare professionals and service providers
- Ensure care aligns with relevant aged care legislation, quality standards and KKCS clinical policies

About You

- Diploma of Nursing (Enrolled Nursing) or equivalent
- Current AHPRA registration
- Experience within aged care, community nursing, community care or a similar environment
- Strong clinical assessment, care planning and documentation skills
- Confidence building relationships with older people, families and other healthcare professionals
- Strong organisation and time management skills
- Current Driver's Licence and access to a reliable, registered and comprehensively insured vehicle
- Current First Aid and CPR
- Urethral and suprapubic catheter insertion and management competency, or willingness to obtain
- Ability to meet KKCS pre-employment screening requirements

Highly regarded:





- Previous Home Care Package or Support at Home experience
- Previous Care Partner/Care Coordinator experience
- Knowledge of aged care funding, budgets and quality requirements
- Wound care and catheter management experience
