About the role

Lead the clinical assessment, care planning and review cycle for a portfolio of Support at Home clients in their own homes. You will coordinate care across a portfolio of clients, sitting at the centre of clinical care, care management, rostering and funding decisions, and reporting directly to senior leadership. This role offers genuine influence over quality, growth and culture, with no ward, no bed count, and no night shifts.

Key responsibilities

- Lead the clinical assessment, care planning and review cycle for a portfolio of SAH clients, including complex care, wound, continence, pain, falls, behaviour, medication and dementia support needs.
- Develop individualised care plans that align clinical need, client goals and the client's approved budget and classification.
- Provide direct nursing care and clinical education to clients, families and carers where required.
- Monitor and evaluate clinical outcomes and deterioration; escalate and adjust care promptly.
- Manage clinical risk for clients identified as at risk, with documented mitigation and follow-up.
- Liaise with GPs, allied health, hospitals, dementia, continence and palliative services to ensure integrated, holistic care.
- Apply working knowledge of Support at Home, classification levels, quarterly budgets, care management funding, Restorative Care and End-of-Life pathways, and co-contribution rules.
- Monitor client budget utilisation and work with Rostering and Finance team to address under- and over-utilisation, unspent funds and service mix.
- Supervise, mentor and clinically support Enrolled Nurses, Personal Care Workers and support staff.
- Develop, deliver and present staff training material to upskill support workers and Enrolled Nurses in areas such as wound care, medication support, manual handling, falls prevention, dementia and behaviour support, infection control, clinical observation and documentation.

About you





- Current registration as a Registered Nurse (Division 1) with AHPRA / the Nursing and Midwifery Board of Australia.
- Substantial experience in aged care, community or in-home care, including clinical leadership and care coordination.
- Working knowledge of the Support at Home program, aged care funding, care management and budget utilisation.
- Strong understanding of the Strengthened Aged Care Quality Standards, accreditation, clinical governance and reportable incident requirements.
- Exceptional clinical assessment, decision-making and risk management skills.
- Demonstrated ability to lead, mentor and motivate a multidisciplinary team.
- Demonstrated experience developing and presenting training material and delivering competency assessments to support workers and Enrolled Nurses.
- Excellent written and verbal communication, with the ability to build rapport across a wide range of cultures and with families, GPs and external providers.
- Current driver's licence, NDIS Worker Screening Check (or willingness to obtain), Police Check and immunisation compliance.
