About the role The Care Partner will act as the primary point of contact for an allocated portfolio of clients and their families. You will be responsible for understanding each participant's individual needs, cultural preferences and goals, developing and reviewing care arrangements, coordinating services, monitoring budgets and outcomes, and maintaining accurate documentation. A key part of this role is building trusted relationships with older clients and their families and helping them navigate the aged care system in a culturally appropriate and accessible way.
Key responsibilities
- Manage an allocated portfolio of Support at Home clients and act as their primary contact person.
- Build strong and professional relationships with clients, families, supporters and representatives.
- Understand and respect clients' cultural, linguistic, religious, dietary and lifestyle preferences.
- Complete client onboarding, care discussions, assessments and regular reviews.
- Develop and maintain individualised care plans based on clients' goals, preferences, assessed needs and available funding.
- Coordinate personal care, domestic assistance, social support, transport, allied health, meals, home maintenance and other approved services.
- Arrange referrals to Occupational Therapists,
Physiotherapists, Dietitians, Podiatrists, Nurses and other health professionals where required.
- Monitor clients' health, wellbeing, functional changes and emerging risks and escalate concerns appropriately.
- Monitor client budgets and expenditure and support clients to make effective use of their Support at Home funding.
- Maintain accurate and timely care notes, assessments, service records and client communications.
About you
- Experience in aged care, home care, community services, healthcare or case management is highly regarded.
- Understanding of the Support at Home program or Australian aged care system is highly desirable.
- Experience working with CALD communities is highly regarded.
- Strong client relationship and communication skills.
- Excellent organisation, follow-up and time-management skills.
- Ability to manage multiple clients and priorities independently.
- Strong written documentation and record-keeping skills.
- Good computer skills and confidence learning CRM and care-management systems.
- Ability to identify risks and escalate concerns appropriately.
- Current National Police Check, Current Victorian driver's licence, Appropriate working rights in Australia.
